Michigan Declares Lack of COVID-19 Vaccination an Underlying Condition: New State Recommendation

Michigan's Chief Medical Executive, Dr. Natasha Bagdasarian, has issued a landmark standing recommendation classifying the lack of a COVID-19 vaccination as an underlying condition for individuals over six months of age. This decision follows Governor Gretchen Whitmer's executive directive prioritizing vaccine access for Michigan residents.

Dr. Bagdasarian's recommendation emphasizes the continued importance of COVID-19 vaccination, particularly for high-risk individuals. While national medical organizations recommend routine vaccination for those at increased risk, Dr. Bagdasarian's directive expands eligibility, ensuring that anyone who desires vaccination can access it. The recommendation aligns with the understanding that COVID-19 vaccines significantly reduce the likelihood of severe illness, hospitalization, and emergency room visits.

The FDA's approval of the 2025-2026 COVID-19 vaccine for individuals 65 and older, and those aged 5-64 with underlying conditions, created a need for clarification regarding the definition of 'underlying condition.' The CDC's list, while not exhaustive, includes factors such as obesity and physical inactivity. Dr. Bagdasarian's recommendation provides that crucial definition within the context of Michigan's public health strategy.

By declaring the absence of a COVID-19 vaccine dose as a qualifying underlying condition, Michigan aims to facilitate broader vaccine access and ensure those who wish to be protected can do so. This innovative approach ensures alignment with the recommendations of organizations like the American Academy of Pediatrics, the American Academy of Family Physicians, and the American College of Obstetricians and Gynecologists. The Michigan Department of Health and Human Services (MDHHS) is actively collaborating with the Department of Insurance and Financial Services and the Michigan Department of Licensing and Regulatory Affairs to guarantee insurance coverage and vaccine availability.
